Healthy Outcomes Through Prevention and Early Support (HOPES)

Caring Support for Stronger Families

Healthy Outcomes Through Prevention & Early Support (HOPES) is a fixable program that offers a variety of innovative initiatives and supports for families with children ages 5 and under. By providing a fixable program, this allows families to participate in any combination of our services. Endeavors HOPES program will include home visiting, parent education, family activities, and emergency assistance for basic needs.

What We Do:

Services are offered to individuals with young child(ren) ages 5 and under and/or expecting parent(s). Please see below for a list of available services:

  • Household must reside in one of the following counties: Matagorda, Wharton, or Brazoria
  • Parenting Education Support Groups and Workshops: Offered in-person
  • Help with Basic Needs: Diapers, formula, canned food, cleaning supplies, etc.
  • Home Visiting Services: In-home support

How does our home visiting service work? Our home visiting program uses the Promoting First Relationships (PFR) model. PFR is a way for parents to gain perspective and confidence. PFR uses a reflective parenting strategy that supports parents in their unique relationship with their child. Our team assists parents in seeing how they can support their child’s emotional health and creates a non-judgmental space to explore and reflect with parents while supporting them in finding their own insights and wisdom.
